Team Mentoring Packet

Team mentoring requires that an applicant's two primary mentors work together to provide mentoring to the applicant (i.e., the mentee). For more information on team mentoring, please refer to the team mentoring contract and the frequently asked questions. The team mentoring packet should include the following:

  1. Signed letters of support from all faculty mentors. There must be a minimum of two primary mentors from different disciplines encompassing a multidisciplinary approach. The primary mentors' letters must state the following with respect to the team mentoring plan:
    • The mentors and mentee will meet on a regular basis. The planned frequency of meetings should be indicated. (In most cases, the primary mentors meet together with the mentee every week. The primary mentors, mentee, and other mentors meet together every month. Alternative meeting plans should be discussed in the letter.)
    • Team mentors will attend meetings of the Multidisciplinary Advisory Committee every 6 months when their mentee is presenting.
    • Team mentors will participate in the 1-day team mentoring training retreat to obtain or enhance skills in team mentoring.
    • Team mentors will participate in biannual evaluations and assessments of the team mentoring relationship.
  2. An NIH biosketch for each mentor.
  3. A list of up to10 individuals whom each mentor has trained or mentored during the past 10 years. The list for each mentor should include the trainees' names, dates, and current positions.
  4. A description of each mentor's past and current research (not to exceed 1 page per mentor).
