Checklist for Submission of the CRSP Application
Please include 1 electronic version, 1 original hard copy version, and 10 collated hard copies of the following:
- An application cover page.
- An abstract of your proposal.
- A table of contents for your proposal.
- A draft budget and budget justification.
- Your NIH biosketch.
- Your CV.
- A description of your background, training, and career goals.
- A description of your career development plans.
- A description of the resources available to you.
- A description of your proposed research project (not to exceed 6 pages).
- A description of the work you will accomplish in 2 years (not to exceed 1 page).
- A human subjects protection plan (not to exceed 1 page).
- A mentoring packet that includes:
- Signed letters of support from all faculty mentors, including at least two primary mentors in different disciplines, and a description of their plans for team mentoring.
- An NIH biosketch for each mentor.
- A list of up to 10 individuals whom each mentor has trained or mentored during the past 10 years, including the trainees' names, dates, and current positions.
- A description of each mentor's past and current research (not to exceed 1 page per mentor).
- A letter of support from your division chief, department chair, or school dean.
- Two letters of reference.
- A personal data sheet.
- A copy of your permanent residency card if you are a non-U.S. citizen.
For the hard copy version, collate the application so that the original and each of the 10 copies is complete. Fasten each copy with one rubber band or one binder clip.
For the electronic version, please put all pieces of the application into one document. This document must be in Word (.doc). Make sure your last name is included in the name of the Word file. (Note: Although letters of support and references letters do not need to be on letterhead or signed for the electronic version, they do need to be signed on the hard copy version.)
All materials will be accepted until 4:00 p.m. on the due date. Send 1 original and 10 collated copies to:
