Application Requirements

Applicants are strongly encouraged to seek advice from and work with the program director, members of the Multidisciplinary Advisory Committee (MAC), faculty, or mentors in the development of their applications.

The application must be in 11-point Helvetica, Georgia, Palatino Linotype, or Arial type with 0.5-inch margins. Combine all parts of the application materials below into a single Word document file. Please sequentially number all of the pages in the proposal (do not use suffixes, e.g., 5a, 5b). Place your name in the upper right-hand corner of each page. The order of the materials is prescribed in the table of contents and is outlined below. Be sure to follow the links indicated for additional descriptions of materials needed. Application processing may be delayed or the application may be returned if it does not comply with all of these requirements.

Application Materials

  1. Cover page.
  2. Abstract that provides an overall description of the proposal (1 page).
  3. Table of contents.
  4. Draft budget and budget justification.
  5. NIH biosketch (note: the biosketch should include grants funded as well as grants pending).
  6. Curriculum vitae.
  7. Description of your background, training, and future career goals (not to exceed 2 pages).
  8. Description of your career development plan, long-term goals, and research project. Please be sure to include your specific aims, with details of the curriculum you wish to pursue and a proposed timeline that explicitly indicates your planned tenure in the CRSP as well as for the K award (not to exceed 3 pages).
  9. Description of the resources that are available to you and will assist you in your research career. Please indicate your affiliation with a university center, institute, or laboratory, and use the PHS 398 form (1 page).
  10. Description of your research project (6 pages). See the proposal instructions.
  11. A brief (1 page) description of the work that you will accomplish in the 2 years of the CRSP. This preliminary research work will serve as the foundation for a career development proposal.
  12. Team mentoring packet.
  13. Letters of support.
  14. Checklist.
  15. Personal data sheet.
  16. A copy of your permanent residency card if you are a non-U.S. citizen.

Appendixes are not allowed for this proposal. A proposal that exceeds the page limitation will be returned to you without review.
